Sim not provisioned

I have an sim card for a phone that hasn't been used for a few months. The number has been reactivated recently but it still says sim not provisioned when I put the sim in my phone. Can you help?

How long ago did you reactivate it? 

Was it on pay as you go or contract? How many months ago has it not been used before reactivating it?

Is this a Pay as you go SIM? 
 

If you haven’t used your phone for any chargeable activity for 179 days (6 months) then your account will go into hibernation and any credit will be removed. After a further 90 days the service will be cancelled.

Are you able to test your SIM card in an alternative handset? 
If you still have the same error message, you'll need to contact Customer Care on 08009566000 so they can check the SIM activation has completed correctly and also the correct SIM card being used/activated
