code is to unlock a SIM card because the wrong Sim pin has been entered three times. You can obtain this via your my EE account or by calling customer services. 

To contact EE Customer Services dial 150 From your EE mobile or 0800 956 6000 from any other phone.

You'll find the PUK, to unblock your SIM after too many incorrect PIN attempts, in your online MyEE under Account Settings / Unlock your SIM.
