If you have access to the app, can you select Manage Add-ons > Inclusive Extras > Manage Inclusive Extra?
It should let you view and change this here.

It showed active for EE and also I was able to use the Internet yesterday because I paid the £5 for 24 hours but I used to have it monthly as part of my plan . But it just disappeared in the 20 days I left and returned
