Forgot a form when sending in phone for repair

I am not an EE customer but needed my phone screen repaired. I had the courier come and collect the phone but forgot to put the form required in with it! how do i contact EE without a phone to try and sort this out?

Yes booked online yesterday and it was collected today.

Also just to be 100% sure, what information is included on the form that you forgot to include here?

Hi,

It just says A courier collection has been requested on the updates page but was picked up yesterday.  It had my personal information and phone information and information about the issue with the phone. I shall keep an eye on the tracking page and see if i can do anything there. Thank you!
