I'm trying to buy a Sim only deal but the form has errors that can't be fixed

Every time I try to go through the check out process for a sim only deal the form comes up with two error boxes at the top of the page. One says I've entered my email wrong and the other says there are errors on the page. First, there isn't even a box to enter your email on that page so I can't 'fix' that and there are no other errors either...! There doesn't seem to be any way to get past this page and buy the sim.

I've already tried using my Windows PC instead of my Mac and also tried changing to a different browser. No change.

Just tried from my mobile which produced a further error that says:

"Please enter your email address. Not a valid email address. 

Your STAC is no longer valid. Please check that you are using the right code from your current network provider."

Followed by 

"Please correct the errors below."

There are no errors. The PAC code is correct and still valid.
