More than 3 users

I have just joined EE from BT.  They could only add 3 out of 4 family members to my account.  They cannot seem to add a fourth.  Has anyone had this issue and how did they solve it?

Did they say why? What happened with the fourth one?

Apparently you cannot have more than 3 users on your bill until they have 3 months of credit history.
