I upgraded my phone and plan 3 weeks ago. But on the app and website although my plan has updated its still showing my old phone and offering me an upgrade!

Do you have an upgrade anytime plan?
If so, this would show as you can Upgrade Anytime from day 15 of a new plan. If you are within contract there would be an upgrade fee.
