The PUK is not phone-specific, but is SIM-specific. It's short-hand for personal unlock key.

It is to unlock a SIM where the PIN has been entered incorrectly a certain number of times. Entering the PUK incorrectly too many times will result in it being permanently blocked.

Perhaps get a friend or relative to cross-check that you're entering the correct PUK.
