I am writing to apply for a replacement SIM card.

Welcome to the EE Community,
If you go into any EE store with photographic ID they will give you a free of charge replacement alternatively you can call customer services on 150 or 0800 956 6000 option 2 then 4 from any other phone and they will arrange for one to be posted to you.
