  • I have obtained a document from EE which is a proof of usage , this confirms the device I lost year year in June is still being used in may 2025 , I need an explanation of why this is so I can provide this to my insurance company

This is not customer services, we have no account access and do not know who you are, you will have to speak to customer services  to get this sorted.

One thing, did you report your phone lost or stolen? EE would have blocked your device and SIM card. If you did not, then not sure how you can prove it.
