Ex employee shortfall letter

I have received a letter in the post stating I am entitled to a shortfall payment as I was part of the 'buy holiday' scheme between 2018-2022 where the pay temporarily dipped below National minimum wage. It's asking me to fill in a form to claim this but no form was included, also the contact email looks a bit suspicious n.pay12@bt.com.  My old employee number and my full name is on the letter.

Could someone please confirm if this is a scam?

I got the same letter and it had my employment number on

 

Is this a scam 😞

If it's got employee numbers on it either it's not a scam or they've had a data breach

I also have had the same letter, also minus the form, but has the correct employee number.

Seems odd the amount of info the letter asks for including N.I number.

What's the best course of action to take?
