I can't find how to setup or mange my spend cap on either the app or web site. I spoke with customer services & they couldn't find it either

You also can only set a spend cap if you have joined or upgraded since Oct 2018.

Hi @john19658.
In your My EE Account, you need to go to Settings, Manage Plan, then Spend Caps.
You can also text SPEND CAP to 150.

Either it's not here or I'm in the wrong place, or My EE account Does not have the correct permissions for me, even though I'm the account holder. Please see attached.

Have you tried texting SPEND CAP to 150?
If you're unable to do this, I'd recommend getting back in touch with our Customer Care team so they can check your My EE Account permissions.
