Despite setting a £5 monthly spend cap my ex fiancee managed to spend over £350 on "services from other companies" . How is this even possible

Spend caps don't restrict charge to bill, for example: in-app purchases.

Also just to add insult to injury I've had my SMS suspended, so I'm locked out of my banking app for failing to verify I am me 😤

It's called a spend cap, to cap your spending to an agreed amount,

Yes, it is but it doesn't cap everything - see How do I set up and manage Spend Cap? :

I dont think your understanding the misinterpretation of the spend cap, it should be called spend visor because the term "CAP" means to cover or seal various situations in there enteririty.
