Paperless billing

I have just had my second bill, and have been charged £4 as an add-on for it.  I noticed this unexpected charge on my first bill and called to request paperless billing.  I was told the first bill is always a paper one, but the next ones would not, so I accepted this. Why am I still being charged?  Do they normally charge £4 a month for online bills as well?  I've never had this with any other network I've been with.

Online bills are free.

Paper itemised bills should be £2.50 pm,
