Can I be sent an itemisers bill from 18 months ago if itemised billing was not set up on my account?

No, itemised billing is not retrospective.
Even a Subject Access Request will only cover the previous 6-12months call history.

You could perhaps submit a Subject Access Request (SAR) by using the online GDPR Form. That should include outgoing calls & texts. It normally takes up to a month but could take up to 3 months.
