Hi I upgraded to a new phone. I am an nhs worker. I was told to fill out the EE perks form which I did and sent over a picture of my ID card. I verified my email and it says they will get back to me. I haven’t heard anything from EE and I need to give them a code within 14 days. Helpppp !!!

Who told you to fill out a form and get a code as you need to put the discount on at point of upgrading.
This can't be done after. I'd give EE a call and see what is happening.

Hi I did it over the phone with an EE member of staff. They said I can give them the code after I receive the device. Within 14 days they said. So I filled out the EE perks phone and confirmed my email but haven’t heard reply from them

Get in contact with EE you can't do it afterwards. You will now probably need to cancel the upgrade and then re-upgrade once they receive the phone back.
But explain the situation to them.
