Locked sim

I've locked my sim then I unlocked it by using my PUK code but now it's locked again and when I put my PUK code it said it's wrong, it asked for a pin but I thought that was the pin

@Macyfinney  A PUK code is to unlock a sim when the wrong sim pin is entered wrong to many times.  EEs default sim pin is 1111.   You can change this on your device to one of your own choosing.       

 What device do you have ?
