Have you tried your sim card in another phone to check if the sim card is what is causing the issue? 

There are different ways you can get a replacement sim card How Do I Swap My SIM or Get a New SIM?

There is no link for ordering for faulty SIMS.

Take phone & old SIM into an EE shop with Photo ID & they'll transfer number, any SIM contacts & credit if applicable to a replacement SIM of correct size for free or phone CS for one. It will come as a Combi-SIM with all 3 sizes as push-outs.
