It looks like you haven't yet linked your SIM to your online account, which is why you don't have the options to manage your SIM- you can see this on the top left of your page.
However, as you can't access your mobile SIM at the moment, you'll not be able to link your product just yet either.
In this situation you'll need to call Customer Service directly, or pop into your local EE store, where a guide will be able to manually help you with your PUK code.
