I've just received my return package how do EE know it's my phone in the package do I have too place  a note in with phone with my name and address on.

Probably best to, with a/c no. Doesn't the return pack have a bar or QR code on it?

The return label has a QR code on it that should be ok

The QR code will have all the required information for the warehouse and the IMEI number will be checked as well making sure this is the correct device you have traded in.
