EE perks Deactivated

Hi, my EE Perks Account has been deactivated as says the following message "Your account has been deactivated as your registration was not completed within the allowed time frame"

I spoke to someone over the phone yesterday but they didnt even know what the NHS perks is... please help 😞

Can I get some help please? My EE Perks account has been deactivated. 
Tried resetting my password but to no avail. It’s saying something about timeframe.
