Since changing to EE (Unlimited plan EU roaming etc) and updating my phone to 26.1 I have been unable to used my iPhone as a hotspot on my MacBook or work iPad or any country including the UK. It connects via bluetooth but says no internet connection. I have turned it off/on, reset network settings etc but no joy.
I have a Airalo E-Sim also which works perfectly fine as a hotspot with any device in any country so it must be an EE sim issue.
Are there any settings which need to be changed to allow me to use the hotspot?

The setting you need is to enable the hotspot on your phone! This may sound flippant, but there shouldn't be anything complicated about it.
Can you use other mobile data-based functions on your phone? Just to prove whether your phone's data connection is the problem, or just the hotspot specifically.

Yes I turn the hotspot on on my phone. As I said using my phone as a hotspot with my other e sim works completely fine.
Yes my phone works perfectly on its own date wise in the UK and abroad.
As I said it’s specifically using the hotspot with my EE sim that I have ‘no internet connection’

I can see in your original message that you mentioned connecting via Bluetooth - is it the same issue if you use WiFi for the hotspot instead?
Also am I correct in thinking that your other device shows as having connected to the hotspot successfully, and it's when you try to actually get online that you're seeing this error?
