We have 9 lines with EE, all on pay monthly contracts, Unlimited Data, Minutes, and Twxts.

If we ring 03457 numbers such as NatWest telephone banking, or Direct Line insurance,  we indeed DO get charged extra. It is not included free with inclusive minutes.

I would be very interested to see a (suitably anonymised) bill where 03 numbers are charged for, on any plan which includes landline calls in the allowance.

That is a serious billing error if it is verifiable.

You sure you are not dialling 08457 for which you will be charged rather than 03457 for which you are most certainly not?
