@dn42 : You need to call CS to report the loss & they will block it & send you a replacement SIM with the same no.

If you no longer know the Apple ID you may need to contact Apple to see if they can help you log into the Find my iPhone site.
Please report the device as lost to us by calling 0800 079 8586.
