my ipad is asking for a data plan

its worked fine until recently - now wants a data plan added via the mobile data tab. asks for a qr code to scan and then add as part of a new 'setup' process.
If I try a manual process is wants
sm-dp+ address
actication code
confirmation code
no idea where to find these.....

Does your iPad have an EE SIM card in it? If so, is it an eSIM?
At what point is it asking for a data plan?

I have owned the ipad for about 2 years, its had the same EE sim in it since I bought it. Its on my own 'owned' plan. It asks for data plan if no wifi availble - data should work and was working before christmas or so.
I have removed and re-installed the sim to be sure the connection is good.
no change - still won't use phone network as a data link.

I'd recommend getting in touch with our Customer Care team so they can check your account and make sure the data plan is still set up. I can't think why it wouldn't be if you haven't made any changes to your plan.
