Hi
We are travelling to Spain next week, then USA End March and South Africa in September.
Do we need to get a new Roaming Package every time or is there a Multi Trip Roaming Pass?

As @Northerner and @bristolian have highlighted, the best roaming packages for your trip will depend on the exact plan you're on, as some of these feature inclusive roaming in select destinations.
In general though, you typically purchase the roaming pass for your destination via a landing page once you arrive, and this will be for a fixed time period, so no need to worry about combining or changing passes.

Roaming passes are zone-specific not country-specific. Purchasing a pass, will provide inclusive usage across all the countries in that zone for the duration of the pass.
As per https://ee.co.uk/roamingrefresh , Spain is in the EU-zone, USA & South Africa both in zone-1. You can purchase individual 7day or 1day passes covering each zone, or look at whether your plan could include them as an optional extra.
Whatever you decide, ensure your phone has the free roaming facility enabled by texting ROAMING to 150 before leaving the UK. This unlocks access to foreign networks and thus coverage abroad.

I tried 150 calling and texting both useless. Impossible to actually talk to anyone.

SIM-only just means your plan doesn't include a phone but just airtime. What's the exact name of your plan? The data allowance isn't relevant, but whether it's (for example) Full Works, All Rounder, Essentials Plus, Essentials or No Frills.
In what way was texting 150 useless? Feel free to post up screenshots of the texts you sent & received.
