EE Perks Not Generating

Hi

I'm trying to get the EE perk code and I keep getting the attached on the photo? I have tried it every day for a few weeks and still not worked.

not sure who I contact to get this sorted?

Well, it says contact your firm's Perks rep. HR probably.
