I'm looking for help to get my wife's SIM deleted. she's got dementia and can't use it any more

Sorry that your wife has dementia, must be a difficult time for you.
Is it for a contract or pay as you go sim card?
If pay as you go, you can just wait until it is automatically cancelled for non use, or you can ring customer service to get this cancelled.
If on monthly contract, you would have to ring customer service giving 30 days notice.

Just adding a little bit to @Schockwave's great reply - If it's a contract, there are some instructions you can follow under the 'ill health' section of this Changing an account in the case of a life event help page.
