I downloaded the QR code thru EE app on separate device, scanned the QR code on the Apple device, I was adding the eSim to. Followed the prompts, final stage stayed at "activating" and the numbers was listed as all "00000000"'s after about 10 minutes, message appeared eSim failed to activate, contact support.

Did you get a text on the old phone? it showing as 0's is normal in my experience.
