Unauthorised charges

Just got my monthly phone bill and found that I have been charged £25 for a roam abroad pass. I didn’t add this and as I haven’t been abroad recently I don’t know how this has been added. I have removed it from my account but I’m not happy that I have been charged for something I didn’t authorise. Anyone any ideas how I can get refunded for this please?
