I am having trouble getting to the 2nd stage of unlocking my phone.

I have filled in the request to unlock my device. I then get email confirmation it has been sent, then I get a 2nd email telling me to go to www.ee.co.uk/appleunlock to activate my unlocking. This just takes me back to the initial unlock request form and I just keep replicating the initial unlock request!

Have you followed the section "How do I set up my iPhone or iPad after it’s been unlocked?" near the bottom? You should not be in the unlock form again.
