I had a similar problem a couple of months ago, where the online form kept saying my phone wasn't an EE phone.
Called 150 and was sorted in less than 5 minutes

Did you buy this device yourself new direct from EE or is it 2nd-hand? The process & costs are different. If you didn't get it direct from EE the online form is no good for you.
