When was the last time you made a chargeable call or text in the SIM? It's likely dead if it's some time. Your PUK code can be obtained from EE.

Thanks for your reply. It has been some time since a chargeable call or text was made. I have been using my work phone but was looking to get a personal phone again and hoped I could use the same number. I'll give customer services a call and see what happens.

If it's been over 269 days since your last chargeable activity, the number will have begun the recycling process and you won't be able to get it back.
If it's under 269 days, the Mobile Care team will be able to reactivate it for you.
