How to get a copy of my sim contract

Hi

 

I too was a customer who had a sim cotract with Orange in 2014. I need to get a copy of my Sim contract I had with ,   then Orange , now EE.  

 

The contract ended up in March 2016. As I lost some documents and the contract papers which are not traceable, I need to get a copy . Please advise.. I will be thankful for you.

Was this SIM card taken out over the phone or in a retail store?

thanks for reply. The SIM card was taken out over the phone and the end of the was in March 2016.

 

I also need just a month's call notes , say April  2015. How can do it ?

 

Should I send subject access request to EE or just call EE and explain ? .

If SAR to be send where can I find the Form ?
