I upgraded my phone by calling the upgrade number, kept the same number etc.
I was able to log on to my ee account online before.
I installed the EE app on the new phone but it keep on saying the password was wrong.
So i tried online, and it set up my phone as a new contract - forgetting about the last one, which i think was 5 years.
is this right?

You should still be able to log in using the same account after an upgrade.
Are you able to see all of the information about your plan, since you made a new account?
