paymant failed error

now i'm using your USIM now.
Before using the EE USIM, i tried to top up several times.
The  phone numebers are XXXXXXXXXXX,  XXXXXXXXXXX.
But this USIM was showed that this page is error several times.....
So i tried to top up to change the credit cards and chek cards.

and then, suddenly i checked my accounts 2017-10-16. 

i shocked when i  see my accounts. 

At that time, The EE run out of moeny several times. 

when i pop up EE USIM,  these cards were error. 

but Your company run out of my money many times.. 

can you tell me why this is happened???
