ee fast replacement fee

I have a Samsung S8 on contract, and have the full cover insurance, around 2 weeks ago i broke my phone and was sent a replacement through the post (£100 fee), after having the phone for 1 week i noticed the touch screen was not working so i rang again and was told on the phone that i would be sent out a different devices (with no charge). but this month my bill has a £48 charge stating fast replacement fee. why is this as i have checked all of the documents for my insurance as nothing says anything about this fee ?
