My mom did the request..she has the ee account..yes sir she bought the phone at and i dont know the terms of payment if its pay monthly or other

As the unlocking form is not working for you it's acting as if the phone is not registered to EE which would happen if it was not bought directly from EE.
If that were the case you would need to call Customer Services on 0800 956 6000 (option 2, option 4) to put you in touch with the Unlocking Team to request an Unlock Code. You will be asked for your email addy. They will take payment (of £8.99) from any valid UK bank card.
Once the process has been started you will receive an email from the Unlocking Team to process the unlock. It can take up to 10 days to complete the unlock.

Surely she must know if she's still paying monthly for it. If she is, then neither her nor you own that phone yet as it's under 6 months old. It's still EE's property. So you can't unlock it.
However, if your mother can discover that she bought it as PAYG, then the 6 month rule doesn't apply & she could unlock it now. It's down to her memory.
