How to find my 14 digit mini SIM serial number?

I got a new EE SIM for my new phone. I'm trying to use this form:

https://tmobile.ee.co.uk/sim-renewal/enter-pin/

 

It says I this:

"You'll find your mini SIM serial number on the envelope your SIM came with, it's 14 digits long."

But I can't find any such number on my envelope.

 

What I want to do is use my old phone number on my new phone (which needed this new smaller SIM). Any help appreciated.

There may be a 12 digit number on the packaging followed by 2 more in ( )s.
