We are having issues using a 3CX telephony app over 3/4G, the provider believes this may be because port 5090 is being blocked, as this works over other ISP's. Is there a possibility this is the case?

No, EE block no ports other than maybe unauthenticated port 25 SMTP & I'm fairly sure they've stopped doing that since they got rid of their own email.
Have your port forwarded it properly? What router are you using?

Inbound won't work due to EE using Carrier Grade NAT (CGNAT). Your provider should be able to advise if it would work or not with this limitation.
