To get the PUK code please log in to My EE, go to Menu >Manage device, scroll down to Unblock SIM (PUK code), select Show PUK code

This doesn’t help! I log into MyEE and there is no option manage device

Follow this link to your Account Settings. The PUK code should be on there.
