I am trying to tether another phone to mine for mobile hotspot. We select the qr code which it recognises as my phone but then still cannot get access to Internet unless using WiFi access which we don't want as trying to avoid roaming charges. What is going wrong?

On Android at least, the QR code is just a shortcut to a WiFi-connection between the host "hotspot" and the recipient device.

If the recipient device is connected to the host device, then that is a WiFi-connection. Can you use mobile data on the device you're using to host the hotspot?
