      <description>&lt;P&gt;Hi, my mother died and my dad had a catastrophic stroke. I need to cancel the phone contracts on their account. The bereavement form doesn't do the job because my dad is still alive.&lt;/P&gt;&lt;P&gt;I could use some suggestions - thanks very much&lt;/P&gt;</description>
