I accidentally sent a picture message and was billed 83p which was listed but also received another charge of £4.99 without any explanation of what this charge is for

It will say on your bill if it is outside your normal charges.

So is this an additional charge as well as charge for a photo message because if so I find this rather excessive

It said extra charges 1/6/23 at 21.41 details blank price £4.99
Then on other charge it says picture message details to the phone number sent and then cost £0.83 total of £5.82

They are 2 separate charges. 83p is the correct charge for an MMS. You'll need to call CS to query the £4.99.
