Spend cap zero, still getting charges

My friend has a zero spend cap and a premium number bar, he assumed his bill was always the same and never checked, but went through it yesterday and found that he was being charged for using a parking service via sms. Its only 15p a go but he assumed he wouldn't of been able to send the message if it cost him

So I guess my question is what isn't included in the spend cap and premium number bar and is still able to use and occur charges?

@DoctorBlue52 : Spend caps do not restrict everything. 1 that is not inhibited is Charge to Mobile Bill. Or it could be a Service Charge to Service Numbers beginning with 09, 118, 083 084, 085, 087, 070. The long list of others can be seen in How do I set up and manage Spend Cap?
