Do you want to:

Unlock your phone to use on other networks (PAC)? OR
Unblock your SIM after trying too many incorrect PINs (PUK)?

You'll find the PUK, to unblock your SIM after too many incorrect PIN attempts, in your online MyEE under Menu > Account Settings > Unlock your SIM.

@Essom : You can just apply for unlocking by using the Unlocking your device form and giving its IMEI no., which you can obtain be dialling *#06# into its keypad.
